Working within a frailty team to visit patients that are housebound living in the community with long term health conditions such COPD, heart failure and asthma.. Help patients to manage their condition, perform venepuncture, observations, inhaler techniques, ECG, collect MSU, stool and sputum samples.

Working in different care and nursing homes assisting nurses and senior care workers to provide person centred care for the elderly.. Assisting with personal care, dressing, eating, drinking and activities.

Working alongside qualified midwives to support mothers and babies after labour.. Duties included doing mother and baby observations, thorough baby checks, breastfeeding support, baby blood tests.. Ensure clinical areas were clean, manage blood spillages, clean and make beds.

Collect patients who were coming in to have surgery done, reassure them and make them feel relaxed before procedure.. Ensure clinical area was cleaned and setup ready for the next patient.

Worked in different settings across the trust which included mental health units, mother and baby units and Poole intermediate care.. Help patients get back to their normal state of daily living.

Supporting young people with emotional and behavioural difficulties to engage with education, life skills and participate in activities within the community.. Contributed to the daily running of the home and completed paperwork such as care plans, risk assessments and reports.

Working in a residential setting supporting young people with autism and learning difficulties with aspects of personal care and daily living tasks within the home.. Help them thrive to their full potential in classroom tasks.

Supporting adults with physical disabilities to live independently as possible in their own homes.. Assisted with daily tasks such as personal care, washing, food shopping, food prep attending appointments etc.

Supporting young people and adults with substance misuse and mental health.. Support to access services, duties to help with paperwork, manage issues and help with accommodation.

Supporting adults with disabilities and mental health, to help them to integrate into the community and access activities.. Help them make choices, friends and independence.
