Case Manager

WE ARE HIRING – SkillUp Workforce Case Manager We are hiring – Case Manager position. Dunklin County Caring Council is seeking a compassionate, dependable, and motivated Case Manager to join our team. The Case Manager will work directly with individuals and families to assess needs, connect them with community resources, develop service plans, maintain required documentation, and provide ongoing support to help clients achieve stability and self-sufficiency. Key Responsibilities: • Conduct client assessments and develop individualized service plans • Provide case management, referrals, and follow-up services • Maintain accurate and timely case files and documentation • Monitor client progress and assist with documentation barriers • Collaborate with community agencies and service providers • Maintain confidentiality with all client data and support requirements Qualifications: • Strong communication, organizational, and interpersonal skills • Ability to work effectively with individuals and families from diverse backgrounds • Ability to maintain professional boundaries and confidentiality • Reliable, detail-oriented, and able to manage multiple cases and deadlines • Experience in case management, social services, or a related field preferred • Join a team committed to strengthening families and making a difference in our community.
