Case Manager

What You’ll Do
* Meet weekly with assigned clients.
* Complete needs assessments and follow up on next steps as clients learn to navigate housing services, Arc, resume building, job search and other aftercare resources.
* Connect clients with community resources.
* Support clients in RTC, PHP, IOP, and Sober Living.
* Document services in our electronic health record.
* Work closely with our clinical team to support client success.

We’re Looking For Someone Who Is:
* A self-starter who works well without constant supervision.
* Organized, dependable, and follows through.
* Compassionate while maintaining healthy professional boundaries.
* Comfortable with computers and learning new software.
* Passionate about helping others grow and succeed.

Case management or behavioral health experience is preferred but not required. Personal recovery experience is valued but not required.

Requirements
* Pass a criminal background check.
* Pass a pre-employment drug screen (UA).
* Hourly pay based on experience.
