Case Manager Specialist

About the role

Peckham, Inc. is hiring a Vocational Services Specialist to provide crisis management, vocational services, case management, and human resources support to individuals with disabilities and other barriers to employment. The role includes vocational counseling and coaching to help team members maintain employment, community resource referrals, and consultation to Peckham staff to support the quality of on-site vocational rehabilitation services.

Responsibilities

• Provide crisis intervention, support, and counseling for participants, and recommend interventions to promote vocational success.
• Provide person-centered vocational support to individuals with disabilities and other barriers to employment.
• Deliver one-on-one counseling, coaching, crisis intervention, or training when needed or requested.
• Identify strategies to help team members work through everyday crises and workplace accommodations that support vocational success.
• Process program-specific intake documents and maintain electronic case files.
• Conduct orientation sessions for new participants.
• Provide person-centered case management services.
• Assist participants in developing individualized program plans based on their vocational goals, strengths, and interests.
