Job description
About the role
The Community Health Representative (CHR) role combines multiple services, including the Community Oral Health Initiative (COHI), Medical Transportation (MST) services, and Maternal Child Health.
Tasks
- Support Health Department staff in delivering health-care services and contribute to team initiatives in health advocacy, health promotion, and illness prevention
- Serve as a liaison between community members and Health Department staff and programs
- Support the delivery of health programs by transporting community members, scheduling Child Health Clinics, and participating in community health events and activities
- Deliver health-promotion initiatives by planning workshops and community education events and developing public-awareness materials
- Promote key health topics at LPLL School and Head Start/Daycare and provide support as needed
