Job description
REQUIREMENTS:
1. Application letter addressed to: REMELY B. LACHICA, DPA
Regional Director
2. Fully accomplished PDS with recent passport size pictures and work experience sheet
3. Performance rating in the last two (2) rating period (if applicable)
4. Authenticated copy of Certificate of Eligibility/ Rating/ License (if any)
5. Photocopy of Transcript of Records / Diploma (if any)
All required documents must be combined into one PDF file to facilitate easier review and reference.
