Hospice Admission Nurse

About the role

As part of our interdisciplinary team, you’ll assess patients, coordinate admissions, and help ensure a smooth start to hospice services.

Tasks

• Conduct comprehensive admission assessments and determine hospice appropriateness
• Develop individualized care plans and provide immediate nursing interventions
• Coordinate medications, DME, and care with physicians and the hospice team
• Educate patients and families about hospice services and benefits
• Support continuity of care across home, hospital, and skilled nursing settings

Requirements

• Current California RN license and CPR certification
• 2+ years of hospice RN experience
• 1+ year of hospice admissions experience
• Valid driver's license and reliable vehicle
• Strong clinical, communication, and problem-solving skills

Pay

• $118,000–$142,000/year
