Key Responsibilities Provide direct patient care including administering medications, monitoring vital signs, and assisting with daily living activities. Maintain accurate patient records and documentation in compliance with healthcare regulations and standards. Educate patients and families on health management, wellness strategies, and post-care instructions. Participate in multidisciplinary team meetings to discuss patient progress and resolve care-related issues. Assist in coordinating healthcare services and resources to optimize patient outcomes. Adhere to safety and infection control policies to ensure a safe environment for patients and staff. Required and Preferred Qualifications Current and valid LPN or LVN license in the state of New Mexico. Strong clinical skills with comprehensive knowledge of nursing practices and procedures. Experience in a healthcare setting, preferably in direct patient care. CPR and First Aid certifications are preferred. Proficiency with electronic health records (EHR) software .
