- Bring photocopies of an updated resume or biodata; NBI, police, or barangay clearance; PSA birth certificate; marriage contract if married; PSA birth certificate for a child; SSS E1, E4, or static; PhilHealth MDR; Pag-IBIG MDF; TOR or diploma; certificate of employment; certificate of competency if skilled; and two 1x1 photos with a red background.
