Job description

About the role

Providing high-quality, personalized care to patients living with Chronic Kidney Disease (CKD) and other complex chronic conditions.

Tasks

Approximately 5–6 patient visits per day
Combination of in-home visits and telehealth
Focus on chronic disease management and preventive care

Requirements

Active Nurse Practitioner license
Board certification
Primary care, internal medicine, geriatrics, or chronic disease management experience preferred
Comfortable providing in-home and virtual care
Valid driver's license and reliable transportation
Bilingual English/Spanish required for Puerto Rico positions

Schedule

Monday–Friday | 8 AM–5 PM (Flexible)
No routine weekends or overnight shifts
Limited after-hours calls
