About the Role
Lead home health patient care services and support a nursing team in delivering compassionate, patient-focused care. You will help ensure regulatory compliance, coordinate personalized care, and advance quality and patient outcomes.
Key Responsibilities
- Lead and manage patient care services in compliance with applicable regulatory standards.
- Supervise, mentor, and support nursing staff to encourage professional growth and positive patient outcomes.
- Work with interdisciplinary teams to develop and implement personalized patient care plans.
- Ensure patient care and progress are documented accurately and on time.
- Audit patient records and care plans to support quality assurance.
- Coordinate communication among patients, families, and healthcare providers.
- Participate in continuous quality improvement and professional development activities.
Qualifications
- Home care experience and two years of management or supervisory experience are preferred.
- Knowledge of professional standards of practice, Medicare Conditions of Participation, and applicable federal, state, and local regulations.
