About the role
N Street Village is seeking a compassionate, clinically skilled, and mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to join its multidisciplinary team in a full-time capacity. The role advances trauma-informed, person-centered, and equitable care for women experiencing homelessness and behavioral health challenges. The PMHNP provides comprehensive psychiatric and nursing care to clients within the Low Barrier Shelter and Wellness Center, supporting client stability, wellness, healing, recovery, and long-term self-sufficiency.
Responsibilities
- Assess physical and behavioral health needs, including thorough physical and mental health evaluations.
- Use evidence-based assessment tools to measure symptoms and functional impairment, formulate diagnoses, and develop individualized treatment plans.
- Monitor client progress and adjust treatment plans as needed.
- Manage mental health conditions, prescribe and monitor medications, and provide therapeutic support, education, and counseling.
- Facilitate individual counseling sessions, support groups, and educational workshops on fostering wellness.
- Implement evidence-based therapeutic interventions to address client needs and symptoms.
- Provide crisis intervention and stabilization for clients in distress.
- Collaborate with healthcare professionals and staff to ensure comprehensive care.
