Social Care Navigator I — Field-Based (Bilingual Spanish)

About the role

Connect Medicaid members with social care services, with a focus on nutritional health, nutrition education and food resource access.

Tasks

• Conduct proactive, face-to-face, and telephonic outreach to Medicaid members across NYC's five boroughs.
• Administer standardized HRSN screenings to identify unmet needs in housing, food security, transportation, and safety.
• Facilitate closed-loop referrals using dedicated technology platforms to ensure members successfully access community resources.
• Manage a dedicated nutrition caseload, connecting members to food pantries, benefit programs, and medically tailored meals.
• Deliver personalized nutrition education, workshops, and community presentations on healthy eating habits and meal planning.
• Represent NYREACH at local community events to expand and strengthen our partner resource network.

Requirements

• High school diploma or GED required.
• Degree in Social Work, Public Health, or Nutrition preferred.
• Experience in social services, community health, care navigation, or a related field.
• Bilingual Spanish (Required) to effectively serve our community members.

Schedule

• Monday–Friday, 9:00 AM – 5:00 PM
• Full-Time, Non-Exempt
• Field-Based / Local Travel Required
