About the role
Xploy Solutions is seeking a compassionate, highly organized, and detail-oriented Senior Medicaid Case Manager. The role supports seniors and their families through the Medicaid application process, combining empathetic client care with analytical review of financial histories, asset statements, and legal documentation. The case manager is accountable for the application file from the initial consultation until approval.
Responsibilities
- Oversee the support team’s financial documentation process and review gathered bank statements, asset disclosures, tax returns, and legal documents covering 3 to 5 years.
- Maintain responsibility for the accuracy and completeness of the application package and ensure compliance with state regulations and deadlines before submission.
- Interview seniors and their families about care needs and financial situations, explaining complex rules in simple, reassuring terms.
- Provide a calm, reassuring presence for seniors who may be confused, stressed, or dealing with cognitive decline, at a pace that helps them feel safe and respected.
- Serve as the primary point of contact between the client, financial institutions, legal representatives, and the Medicaid office to resolve queries or requests for additional information.
