About the role

Xploy Solutions is seeking a compassionate, highly organized, and detail-oriented Senior Medicaid CaseManager. The role supports seniors and their families through the Medicaid application process, with responsibility for reviewing the application file from the initial consultation until approval.

Responsibilities

• Oversee the financial documentation process managed by the support team.
• Review 3 to 5 years of gathered bank statements, asset disclosures, tax returns, and legal documents to understand the client's financial profile.
• Maintain responsibility for the accuracy and completeness of the application package, ensuring compliance with state regulations and deadlines before submission.
• Interview seniors and their families about care needs and financial situations, and explain complex rules in simple, reassuring terms.
• Provide a calm, reassuring presence for seniors who may be confused, stressed, or dealing with cognitive decline, moving at a pace that keeps them feeling safe and respected.
• Act as the primary point of contact between the client, financial institutions, legal representatives, and the Medicaid office to resolve queries or requests for additional information.
