Home-Based Case Manager

Provide home-based case management services. Coordinate and monitor individualized service plans. Connect clients with community resources and support services. Maintain accurate documentation and case notes. Conduct regular home and community visits. Collaborate with families, providers, and community partners. Provide supervised visitation services in accordance with agency policies and court or program requirements.

Qualifications:
Excellent communication and organizational skills.
Reliable transportation.
Ability to work independently.
Experience in social services, human services, or a related field preferred.
