Key Responsibilities Perform thorough physical assessments and review medical histories to determine patient conditions. Create and implement individualized treatment plans tailored to each patient’s rehabilitation goals. Deliver therapeutic exercises and training to enhance patients’ functional abilities and independence. Educate patients and family members on injury prevention, wellness, and recovery processes. Maintain detailed and accurate documentation of patient progress and outcomes. Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care. Qualifications Graduated from an accredited Physical Therapy program. Minimum of nine months experience in Home Health physical therapy. Valid Physical Therapist license in Texas. CPR certification required. Excellent interpersonal skills with a compassionate, patient-centered approach.
