Educate patients and families on therapy techniques and home exercises to support recovery. Maintain accurate, timely documentation of patient care in compliance with healthcare regulations. Collaborate with healthcare professionals to ensure coordinated, holistic patient care. Engage in ongoing professional development to stay current with best practices in physical therapy and home healthcare. Required and Preferred Qualifications Valid South Carolina physical therapy license. Certification in Home Healthcare preferred. Strong patient assessment and treatment planning skills. Experience with diverse patient populations, including chronic conditions. Proficiency with electronic medical records (EMR) and telehealth platforms. Minimum 2 years of physical therapy experience, ideally in home healthcare.
