Registered Nurse (RN) – Home Health Nurse in Houston, TX

Kiwi Consulting Group is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team in Houston, TX . This full-time, field-based nursing role offers the opportunity to deliver personalized, patient-centered care directly in patients’ homes, making a meaningful impact within the community. Key Responsibilities Perform comprehensive clinical assessments to evaluate patient health and care needs. Develop, implement, and update individualized care plans collaboratively with patients, families, and healthcare teams. Administer medications and conduct medical procedures including IV therapy and wound care. Document patient progress and modify care plans based on health status changes. Coordinate care and communicate effectively with multidisciplinary teams to ensure seamless continuity of care. Provide patient and family education on health management and treatment options. Maintain compliance with regulatory standards and best nursing practices. Required and Preferred Qualifications Bachelor of Science in Nursing (BSN) degree required. Active Registered Nurse (RN) licensure in Texas. Clinical expertise in patient care, wound care, and IV therapy. Experience in Home Health nursing or related clinical settings preferred. Proficient in electronic documentation and care coordination. Excellent interpersonal and communication skills. Minimum 1-2 years of clinical nursing experience.
