Key Responsibilities Conduct thorough patient assessments including history, physical exams, and diagnostic testing. Develop and implement personalized treatment plans tailored to patient needs. Collaborate with multidisciplinary healthcare teams to optimize patient outcomes. Manage follow-up care ensuring continuity and quality of care. Monitor patient progress and adjust treatment plans as needed. Required and Preferred Qualifications Master's Degree in Nursing from an accredited institution. Fluent bilingual communication skills in English and Spanish. Proficiency with NextGen EMR or equivalent electronic medical record systems. Strong clinical skills in assessment, diagnosis, and treatment planning. Minimum 2 years clinical experience preferred. Certifications such as ANCC or AANP highly recommended. Commitment to patient-centered care and clinical excellence.
