Care Coordinators

We're looking for organized, reliable, and proactive Care Coordinators to support our home care operations and help us deliver high-quality care to our clients and communities.

KEY RESPONSIBILITIES:
- Coordinate, schedule, and monitor daily home care services and caregiver assignments.
- Serve as the main point of contact for clients, families, and caregivers to handle inquiries and resolve operational changes.
- Maintain accurate, up-to-date client files and care schedules in compliance with agency standards.
- Prioritize multiple tasks efficiently in a fast-paced environment.
- Collaborate with internal teams to drive operational efficiency and high client satisfaction.

QUALIFICATIONS:
- Proven experience working in a home care setting and in a Care Coordinator role.
- Exceptional organizational skills and strong attention to detail.
- Excellent written and verbal communication skills with a proactive mindset.
- Strong team player with a dependable, adaptable work ethic.

Compensation: $23.00–$27.00/hr (Flexible pay rate based on experience)
