About the role
Chesapeake Connections helps people achieve success in their community through mental health services and support programs. The team-based approach offers creative solutions for clients’ needs and supports success in treatment or recovery. This role coordinates and provides comprehensive care and support.
Schedule: Monday - Friday, 8:30am to 4:30pm, with on-call flexibility. Job location type: In Person.
Responsibilities
- Coordinate services for assigned clients, including psychiatric and medical care, housing, entitlements, and more.
- Develop individualized service plans with clients based on their goals.
- Provide ongoing and supportive assistance to clients.
Requirements
- High school diploma with 3-5 years of experience or a bachelor’s degree in a mental health related field.
- Driver’s license with 3-points or less and access to an insured vehicle.
- Availability for on-call hours, as this role may require it.
