Case Manager, Community Treatment

Schedule: Monday - Friday, 8:30am to 4:30pm (requires on call flexibility)

This is an opportunity to support the mental health of your clients by coordinating and providing comprehensive care and support.

• coordinate services for assigned clients, including psychiatric and medical care, housing, entitlements, and more
• develop individualized service plans with clients based on their goals
• provide ongoing and supportive assistance to clients

what we need from you

• high school diploma with 3-5 years of experience or bachelor’s degree in mental health related field
• a driver’s license with 3-points or less and access to an insured vehicle
• availability for on-call hours as this role may require it
