Case Manager RN

Skills, key job functions include assessment of identified members, development of a safe discharge plan from acute inpatient, skilled nursing, LTAC and Inpatient Rehab facilities. Requirements: 2+ years of Case Management experience in Hospital, GA or Compact RN License, BLS from AHA. Duration: 13 weeks contract. Shift: 08:30 AM to 05:00 PM. Weekly Hours: 40. Pay Package for 36hrs: Gross Weekly: $3030, W2 Hourly: $29/hr, Travel/Meals & Incidentals: $1870, OT: $90/hr.
