Intake Coordinator

Heart to Heart Hospice of Houston is hiring an experienced Intake Coordinator!

We are looking for someone who can confidently support the referral and intake process while providing excellent service to patients, families, referral partners, and our internal teams.

We’re especially looking for candidates with:
-3–5 years of healthcare experience, preferably in hospice operations
-Admissions/intake and customer service experience
-Experience working with EMR systems
-Knowledge of healthcare regulations, payer practices, and operational processes
