Position Overview
Priority Group is seeking a Spanish- and English-speaking Master Social Worker (MSW) to provide Comprehensive In-Home Care (CIC) and/or Independent Living Skills Training (ILST). You will work directly with individuals to develop and implement person-centered plans that reflect their goals and preferences while supporting independent living and community participation.
Responsibilities
- Conduct initial and ongoing assessments of each individual's needs, preferences, strengths, and challenges.
- Develop and implement person-centered plans that support individual goals, independence, and community integration.
- Provide ongoing coaching and support in clients' homes or community settings, as outlined in their plans.
- Assist with daily living activities, which may include meal preparation, medication management, and personal hygiene.
- Train individuals and families in budgeting, household management, communication, and related independent living skills.
- Coordinate care with other providers, including healthcare professionals, therapists, and case managers.
- Document services and progress in accordance with program and agency requirements.
Qualifications
- Master of Social Work (MSW) degree.
- Fluency in Spanish and English.
- Ability to provide person-centered support in home and community settings.
- Strong assessment, communication, documentation, and care-coordination skills.
Compensation and Schedule
