Position Overview
We are seeking a compassionate and skilled Master Social Worker (MSW) fluent in Spanish and English to provide Comprehensive In-Home Care (CIC) and Independent Living Skills Training (ILST). This role involves working directly with individuals to create and implement person-centered plans that support their goals, preferences, and promote autonomy within their communities.
Key Responsibilities
- Conduct initial and ongoing assessments to evaluate individual needs, strengths, preferences, and challenges.
- Develop and implement customized person-centered plans that encourage independence and community integration.
- Provide continuous support and coaching in home or community settings as outlined in care plans.
- Assist with daily living activities including meal preparation, medication management, and personal hygiene.
- Deliver training to clients and families on budgeting, household management, and effective communication.
- Coordinate care with healthcare professionals, therapists, case managers, and other service providers.
Qualifications
- Master of Social Work (MSW) degree required.
- Bilingual proficiency in Spanish and English is mandatory.
- Experience in home healthcare or community-based social work preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
