Nurse Practitioner Diabetes Hypertension Program

This community-based role focuses on improving outcomes for health plan members living with diabetes and uncontrolled hypertension. You will meet patients where they are — including in their homes — to provide education, clinical support, and ongoing care. Responsibilities include: Completing in-home member visits, Educating members on diabetes, hypertension, medications, nutrition, and self-management, Managing and titrating medications within scope and established protocols, Identifying and closing gaps in care, Supporting members with uncontrolled chronic conditions, Coordinating care with health plans, PCPs, and interdisciplinary teams. We are seeking NPs with strong experience in diabetes and hypertension management, confident working independently and driven by meaningful patient outcomes.
