Responsibilities
- Assist individuals seeking eligibility for housing subsidy to transition from an institutional setting to the community.
- Work as part of a coordinated Independent Living service team.
Requirements
- Associate or Bachelor’s degree in Human Services or a related field preferred.
- Housing, Independent Living, long-term care, or disability experience preferred.
- Strong communication, organization, and computer skills.
- Reliable transportation required.
