Jobs at WNYIL is seeking an Open Doors Transition Specialist to assist individuals in re-entry from institutional settings to the community.
Responsibilities
- Facilitate and coordinate a comprehensive plan for individuals residing in institutions to re-enter the community.
Requirements
- An associate or bachelor’s degree in human services or a related field is preferred.
- Experience with long-term care, disabilities, or community-based services is preferred.
- Knowledge of Medicaid and 1915(c) waivers.
- Strong communication and organizational skills.
- Reliable transportation is required.
