Job description
Assists individuals in re-entry from institutional settings to the community. Facilitate and coordinate, for individuals residing in institutions, a comprehensive plan to re-enter the community.
Qualifications:
• Associate or Bachelor’s degree in Human Services or related field preferred
• Experience with long-term care, disabilities, or community-based services preferred
• Knowledge of Medicaid and 1915(c) waivers
• Strong communication and organizational skills
• Reliable transportation required
