$23/hour

Initial OTC Coverage:
First 2 weeks require 8-hour coverage:
• 2 Weekdays, 1 Weekend shift, 11:00am - 7:00pm
• Mon, Wed, and Sat; or Tue, Thu, Sun; or Wed, Fri, and Sun; depending on the need; and a weekend day.

Client Information:
Female, #1276572026

Care Needs:
Advanced dementia, chronic constipation, severe mobility impairment, and CPAP use during sleep.

ADLs & Responsibilities:
- Full assistance with ADLs and personal care
- Assist with meals and fluids
- Wheel chair to the dining area and assist as needed
- Use Hoyer lift for safe transfers
- Reposition and provide skin protection
- Assist with stretching and ROM as tolerated
- Provide daily one-on-one stimulation and activities
- Assist client with facility activities
- Monitor bowel movements, sleep/CPAP tolerance, mood, and behavior
- Maintain continuous supervision and safety
- Report incidents, concerns, and changes from baseline
- Maintain client’s privacy, dignity, and confidentiality
- Provide real time care update with timestamp
