Home Health Physical Therapist - Ladson, SC

Projukta Bhattacharya is hiring a full-time Home Health Physical Therapist in Ladson, South Carolina. In this in-home healthcare role, you will provide individualized physical therapy to help patients improve mobility, regain independence, and enhance their quality of life. Pay is $65 per hour.

Key Responsibilities

• Assess patients’ physical therapy needs and develop individualized treatment plans.
• Deliver therapeutic exercises and interventions to improve mobility and function.
• Track patient progress and update treatment plans to support optimal outcomes.
• Teach patients and families therapy techniques and exercises that promote recovery.
• Document care and progress accurately and promptly in accordance with healthcare regulations.
• Coordinate with nurses, occupational therapists, and other healthcare professionals to support comprehensive patient care.
• Participate in continuing education and training on physical therapy and home healthcare best practices.

Required and Preferred Qualifications

• Valid South Carolina physical therapy license.
• Home healthcare certification.
• Strong patient assessment and treatment-planning skills.
• Experience serving diverse patient populations, including people with chronic conditions.
• Proficiency with electronic medical record (EMR) systems and telehealth applications.
• At least two years of physical therapy experience; home healthcare experience is preferred.

Team and Work Setting

This full-time, in-person position provides care in patients’ homes in the Ladson, SC area. You will report to the Clinical Manager and collaborate with a supportive multidisciplinary team committed to professional growth and coordinated care.

Compensation

$65 per hour
