Job description
About the role
Park Nicollet Health Services is looking to hire a Physical Therapist to join its Home Care/Hospice team. The Homecare Physical Therapist acts as a case manager, assesses and evaluates patient care needs, and provides skilled therapy care and teaching to patients and families in their home. The role supports patients and families in preparing for self-directed and/or assisted care in their community setting.
Work schedule: Monday–Friday, 8 a.m.–4:30 p.m. Occasional weekends.
Responsibilities
- Assess and evaluate patient care needs.
- Provide skilled therapy care and teaching to patients and families in their home.
- Improve and prepare patients and families for self-directed and/or assisted care in their community setting.
- Coordinate medical, social, and financial resources to develop a comprehensive plan of care.
- Support admission, discharge, referrals to other disciplines, billing, and care delivery.
Requirements
- Bachelor’s, master’s, or doctorate degree in Physical Therapy.
- Current license with Minnesota Board of Physical Therapy.
- Current BLS certification for health professionals through the American Heart Association or Red Cross, maintained. Online BLS certification is not acceptable.
- At least 3 years of work experience as a Physical Therapist preferred.
- Experience in homecare desirable, with knowledge of OASIS data set.
