Physical Therapist - Homecare

Park Nicollet is looking to hire a Physical Therapist to join our Home Care/Hospice team in St. Louis Park! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
The Homecare Physical Therapist acts as a case manager to access and evaluate patient care needs and provides skilled therapy care and teaching to patients and families in their home. The Physical Therapist is responsible to improve and prepare patients and families for self-directed and/or assisted care in their community setting. The Physical Therapist coordinates medical, social, and financial resources to develop a comprehensive plan of care. The therapist supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery.
Work Schedule: Monday – Friday 8a-4:30p. Occasional weekends.
Required QualificationsBachelors, Masters, or Doctorate degree in Physical TherapyCurrent license with Minnesota Board of Physical TherapyCurrent BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable.Work experience as a Physical Therapist for at least 3 years preferredExperience in homecare desirable with knowledge of OASIS data set.
