Job description
Pennie's Home Health is hiring licensed Physical Therapists and Occupational Therapists for full-time home health roles in Fort Lauderdale and Broward County, Florida.
Provide in-home therapy to help patients restore function, improve independence, and enhance quality of life. As part of an interdisciplinary care team, you will assess patients, create individualized treatment plans, and coordinate care with physicians, caregivers, and other professionals.
Responsibilities
- Conduct comprehensive evaluations and assessments in patients' homes.
- Develop and implement individualized care plans in accordance with physician orders.
- Deliver therapeutic treatments that support recovery, function, and independence.
- Monitor and document patient progress, reactions, and changes in condition.
- Teach patients and families about therapy goals, exercises, safety, and equipment use.
- Coordinate with nurses, caregivers, and other professionals, including through case conferences.
- Complete accurate, timely documentation that meets agency and regulatory standards.
- Maintain current knowledge through continuing education and agency meetings.
Qualifications
- Current Florida license as a Physical Therapist or Occupational Therapist.
- Graduate of an accredited physical therapy or occupational therapy program.
- NBCOT certification is required for internationally educated Occupational Therapists.
- Knowledge of orthopedic, neurological, and post-surgical rehabilitation.
- Familiarity with adaptive equipment, mobility aids, and home safety.
- Compassionate, patient-centered approach and strong communication skills.
