Through Maryland Medicaid, the company helps family members provide paid, in-home care to their loved ones, enabling them to age safely and comfortably at home.

Tasks

• Conducting comprehensive nursing assessments in the home
• Developing and updating individualized care plans
• Perform supervisory visits and competency assessments
• Ongoing monitoring of client progress and safety
• Lead a team of caregivers through on-site training and ongoing supervision

Requirements

• Active Registered Nurse (RN) license in the state of Maryland
• Demonstrated clinical skills in assessment, care planning, medication management, and ongoing monitoring in a home or community-based setting.
• Experience or strong interest in home care, geriatric nursing, chronic disease management, and supporting aging in place.
• Knowledge of Medicaid waiver programs, home health regulations, and documentation standards or willingness to learn these requirements.
• Self-starter who thrives in a dynamic, mission-driven start up environment
• Strong communication and interpersonal skills, including the ability to educate clients and families and collaborate with interdisciplinary teams.
