Join Pennie's Home Health as a Registered Nurse (RN) in Fort Lauderdale, Florida. We are seeking a compassionate and experienced RN to provide exceptional home health care services. Qualifications / Educational Requirements: Must be a Registered Nurse (R.N.) with a current, valid license. Minimum of one year nursing experience as an RN. Ability to work positively and collaboratively with patients, families, and healthcare staff. Responsibilities / Essential Functions: Conduct initial home care patient visits and regularly re-evaluate patient needs and progress. Initiate and implement the plan of care following physician orders. Complete admission, transfer, re-certification, resumption of care, and discharge OASIS documentation. Observe, assess, and document patient symptoms and monitor reactions and progress. Educate patients and caregivers on disease processes, medications, individualized treatment plans, and home care techniques. Coordinate patient services and supervise Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs). Notify physicians and clinical team members of any changes in patient condition. Adhere strictly to Medicaid and Medicare regulations and agency policies. Discharge patients from skilled nursing services when criteria are met. Participate in case conferences to ensure coordinated care. Maintain and update clinical records according to agency standards. Engage in ongoing professional development through in-service training, continuing education, and seminars. Work Environment and Physical Requirements: This role requires travel to patient homes within Fort Lauderdale, Florida. The RN must be able to perform physical tasks associated with patient care in home settings. Employment Type: Full-time position, on-site, not eligible for remote work.
