We are seeking a compassionate and experienced Registered Nurse to provide exceptional home health care services. This full-time role requires a current RN license and at least one year of nursing experience. You will work closely with patients, families, and healthcare staff to deliver personalized care in patients' homes.

Qualifications and Educational Requirements:

  • Current Registered Nurse (R.N.) license.
  • Minimum one year of nursing experience as an RN.
  • Positive and professional interaction with patients, families, and staff.

Responsibilities and Essential Functions:

  • Conduct initial home care patient visits and regularly re-evaluate patient needs and progress.
  • Implement the plan of care as prescribed by physicians.
  • Complete admission, transfer, re-certification, resumption of care, and discharge OASIS documentation.
  • Observe, assess, and document patient symptoms and progress.
  • Monitor patient reactions and adjust care plans accordingly.
  • Educate patients and caregivers on disease processes, medications, and individualized treatment plans.
  • Train patients and caregivers on home health care techniques.
  • Coordinate patient services and supervise Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs).
  • Communicate changes in patient condition to physicians and clinical team members.
  • Follow agency policies and procedures for all nursing skills.
  • Discharge patients when skilled nursing criteria are met.
  • Participate in case conferences to ensure coordinated care.
  • Maintain and update clinical records according to policy.
  • Engage in continuous education through in-service programs, seminars, and self-study.
  • Ensure compliance with Medicaid and Medicare regulations.

Work Environment and Physical Requirements:

This role involves visiting patients in their homes throughout Fort Lauderdale. The nurse must be able to perform physical tasks associated with patient care and travel between locations. Reasonable accommodations may be provided.
