Coordinate daily patient services and caregiver schedules, helping maintain continuity of care and timely service delivery.. Monitor caregiver clock-in and clock-out activity, Electronic Visit Verification (EVV), and attendance records using HHAeXchange.. Review service authorizations and coordinate schedules in accordance with approved hours, service dates, and patient needs.. Manage schedule changes, caregiver call-outs, missed visits, replacement requests, and service resumptions.. Communicate with patients, caregivers, family members, and internal teams to clarify service needs and resolve operational issues.. Follow up on incomplete timesheets, clocking discrepancies, system access concerns, and documentation gaps.. Record case updates and service-related communications clearly and accurately.. Coordinate notifications involving hospital stays, medical appointments, temporary service interruptions, and changes in care arrangements.. Prepare professional written communications in English and Spanish while supporting a diverse patient and caregiver population.
