Blended Case Manager BCM

BCM will promote resiliency and recovery for children, adolescents, adults and their families by accessing, coordination, mentoring, and advocating for community resources and services.

QUALIFICATIONS
- Bachelor's Degree in Human Services or related field
- 40 hours per week
- Flexible schedule; on-call is required
- Blended Case Management experience preferred

+ $2,000 Sign-On Bonus
