Key Responsibilities Perform thorough assessments of patients’ health status to develop and implement individualized care plans that meet their unique needs. Administer medications and treatments accurately as prescribed by physicians, ensuring patient safety and adherence to protocols. Monitor patient progress closely, documenting changes meticulously and communicating updates effectively with families and interdisciplinary care teams. Educate patients and their families on health management, medication adherence, and self-care techniques to promote independence and prevent complications. Collaborate with healthcare professionals including therapists, social workers, and physicians to ensure comprehensive, holistic care and optimal patient outcomes. Assist patients with daily living activities such as bathing, grooming, and mobility support when necessary, enhancing their comfort and dignity. Participate actively in quality improvement initiatives aimed at elevating care standards and increasing patient satisfaction within our home care services. What We're Looking For Required: Associate Degree in Nursing (ADN) from an accredited institution. Active and valid nursing license in the state of Florida. Minimum of 1-2 years nursing experience, preferably in home care or similar healthcare settings. Proficiency with electronic medical record (EMR) systems and healthcare software. Strong compassion, empathy, and excellent communication skills to build trust with patients and families. Ability to work independently, manage time effectively, and adapt to dynamic patient needs. Preferred: Experience in home health nursing or community-based care. Additional certifications related to home care or chronic disease management.
