Clinical Pharmacist

Key Responsibilities: - Conduct comprehensive medication reviews to ensure safe and effective drug use. - Provide personalized patient counseling on medication adherence and side effect management. - Collaborate with healthcare providers to develop and adjust individualized medication therapy plans. - Monitor patient outcomes and intervene promptly to optimize therapeutic results. - Educate patients and clinical staff on new pharmaceutical developments, drug interactions, and safety protocols. - Maintain accurate, up-to-date medication records and documentation in compliance with regulatory standards. - Participate actively in interdisciplinary team meetings to contribute expert pharmaceutical insights. - Stay current with clinical pharmacy best practices and emerging research to continually improve patient care. Qualifications: - Bachelor's Degree in Pharmacy; PharmD preferred. - Current valid pharmacist license in Delaware. - 2-5 years of experience in clinical pharmacy settings. - Strong clinical skills with a patient-centered approach. - Proficiency with electronic health records (EHR) and pharmacy management software.
