Key Responsibilities Develop and manage comprehensive patient care plans tailored to individual needs in home health and facility settings. Collaborate effectively with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated care delivery. Provide expert symptom management and support for patients with serious, chronic, or terminal illnesses. Educate patients and families about disease progression, treatment options, and advance care planning. Document patient progress and clinical interventions accurately in compliance with regulatory standards. Participate in quality improvement initiatives to enhance patient outcomes and service delivery. Maintain compliance with Pennsylvania state nursing regulations and organizational policies. Support patients and families emotionally and psychologically, fostering a compassionate care environment. What We're Looking For Required: Master's degree or higher in Nursing with Nurse Practitioner certification. Minimum of 2 years clinical experience, including at least 1 year in hospice, palliative care, or home health settings. Active Nurse Practitioner (NP) and Registered Nurse (RN) licenses in Pennsylvania. Valid American Heart Association Basic Life Support (AHA BLS) certification; Advanced Cardiac Life Support (AHA ACLS) preferred. Strong communication and interpersonal skills with a patient-centered approach. Ability to work independently and as part of a multidisciplinary team. Preferred: Experience with electronic health records (EHR) and clinical documentation systems. Additional certifications in palliative care or hospice nursing.
