Home Health Physical Therapist
Location: Elizabethtown, Pennsylvania, serving Lancaster, Lebanon, Dauphin, and Harrisburg
Employment: Full-time, direct hire
Pay: $70.00–$89.00 per visit
Meraki7 Inc is seeking a compassionate, skilled Physical Therapist to provide personalized home health care in the Elizabethtown area. Help patients improve mobility, recover function, and work toward their health goals in the comfort of their homes.
Responsibilities
- Assess patients and develop individualized treatment plans.
- Deliver physical therapy interventions to support recovery, mobility, and quality of life.
- Collaborate with a multidisciplinary care team.
- Teach patients and families exercises and techniques to support ongoing health.
- Track patient progress and adjust treatment plans as needed.
Qualifications
- Valid Pennsylvania Physical Therapist license.
- Compassionate approach and strong interpersonal skills.
- Excellent communication and organizational abilities.
- Home health experience is a plus, but not required.
