Role: Physical Therapist - Home Health Location: Elizabethtown, Pennsylvania (serving Lancaster, Lebanon, Dauphin, Harrisburg) Employment Type: Full-Time Direct Hire Pay Range: $70.00 - $89.00 per visit (Pay-Per-Visit) Are you a compassionate and skilled Physical Therapist eager to provide personalized care in patients' homes? Join Meraki7 Inc’s home health team in Elizabethtown, PA, and help patients improve mobility and quality of life through tailored physical therapy interventions. Key Responsibilities: Perform comprehensive patient assessments to develop individualized treatment plans. Deliver physical therapy treatments aimed at recovery and mobility enhancement. Collaborate with multidisciplinary healthcare professionals to ensure holistic patient care. Educate patients and families on exercises and health maintenance techniques. Track patient progress and modify treatment plans as needed. Qualifications: Valid Physical Therapist license in Pennsylvania. Strong interpersonal and communication skills with a compassionate patient approach. Organizational skills and ability to work independently. Home health experience preferred but not required.
