Meraki7 Inc is hiring an experienced Home Health Physical Therapist to provide in-home rehabilitation services in the Brenham and College Station communities. Help patients improve mobility, functional ability, and quality of life through individualized care in the comfort of their homes.

Key Responsibilities

• Perform comprehensive physical assessments and review patients’ medical histories.
• Develop and implement personalized treatment plans based on patients’ rehabilitation goals.
• Provide therapeutic exercises and functional training to support independence and recovery.
• Educate patients and families about injury prevention, wellness, and recovery.
• Document care and monitor patient progress and rehabilitation outcomes.
• Collaborate with a multidisciplinary care team to support coordinated patient care.

Qualifications

• Graduate of an accredited physical therapy program.
• At least nine months of professional experience as a Physical Therapist.
• Previous home health care experience required.
• Valid Texas Physical Therapist license.
• Current CPR certification required.
• Strong interpersonal skills and a compassionate approach to patient care.
