ROLE - Physical Therapist- Home Health

Location: Brenham and College Station, TX

Type: Direct Hire

Pay Range: $105K–$115K/Year

Key Responsibilities

• Conduct comprehensive physical assessments and review medical histories to evaluate patient conditions.
• Develop and execute personalized treatment plans that align with each patient's unique rehabilitation goals.
• Administer therapeutic exercises and training to improve patients' functional skills and independence.
• Educate patients and their families on injury prevention, wellness strategies, and recovery steps.
• Maintain accurate documentation to track patient progress and rehabilitation outcomes.
• Collaborate with a multidisciplinary team to ensure holistic and effective patient care.

Qualifications

• Graduation from an accredited Physical Therapy Program.
• Experience in Home Health care is required.
• A minimum of nine months of professional experience as a Physical Therapist.
• Valid state license as a Physical Therapist.
• Outstanding interpersonal skills with a compassionate approach to patient interactions.
• CPR certification is required.
