About the role

Signature Health, Inc. provides integrated healthcare for the community, specializing in patients with mental illness and/or addiction. The Qualified Mental Health Specialist (QMHS) reports to the Supervisor and Manager, Behavioral Health Services, works primarily in the community, and helps develop and carry out client treatment plans.

Responsibilities

• Develop, prepare, and present treatment program work material to clients.
• Plan, coordinate, and provide reliable transportation so clients can access resources identified in their treatment plans.
• Maintain clear communication and professional boundaries with clients.
• Support clients in accessing and developing a sober support network.
• Collaborate with a client’s treatment team.
• Identify support needs for adult and child clients (SMD/SED, CD/Dual DX) and provide ongoing community support resources to clients and their families.
• Foster positive relationships with clients, their families, and Signature Health team members.
