Home Care Community Outreach Referral Coordinator

About the role

Violetrsa is a growing home care agency seeking a part-time Home Care Community Outreach Referral Coordinator to help connect families with non-home support. The role focuses on building relationships, creating opportunities, and serving as a trusted community resource.

Work 2 days per week, 5–8 hours per day, with local travel throughout Maricopa County, AZ.

Responsibilities

• Conduct in-person, telephone, and email outreach.
• Develop relationships with potential referral and community partners.
• Help develop and follow up on new and pending referral opportunities.
• Maintain accurate outreach and referral records.
• Represent Violetrsa professionally in the community.

Requirements

• Experience in community outreach, networking, sales, referral development, healthcare, social services, or a related field.
• Strong communication and relationship-building skills, including email and in-person communication.
• Valid driver’s license and reliable transportation; ability to travel throughout Maricopa County.
• Self-motivated, organized, dependable, and comfortable working independently.
• Ability to document outreach activities and follow up consistently.
• Comfortable making phone calls, visiting organizations, and developing professional relationships.
• Preferred: knowledge of AHCCCS/ALTCS; experience with healthcare, home care, or social-service organizations; existing relationships with community organizations or healthcare professionals; or previous self-development or experience working independently.
