Participants enjoy social activities, supervision, and assistance tailored to their individual needs. The center focuses on fostering dignity, social connection, and overall well-being for each attendee.Role Description The Intake Coordinator is a full-time, on-site position based in Woodmere, NY, responsible for managing the intake process for new and prospective participants. This role includes responding to inquiries, conducting initial assessments, gathering and verifying required documentation, and coordinating admissions in alignment with program and regulatory requirements. The Intake Coordinator will maintain accurate records, update participant information, and collaborate with program staff to ensure a smooth transition into services. Daily tasks also involve communicating with families, caregivers, healthcare professionals, and community organizations, as well as supporting ongoing quality and service improvements.Qualifications Demonstrated ability to manage client intake processes, including assessment, documentation, and coordination of services.Strong communication and interpersonal skills for working with participants, families, caregivers, and interdisciplinary teams.Organizational and time-management skills to handle multiple cases, maintain accurate records, and meet deadlines.Comfort with basic computer applications and electronic record systems; ability to learn new software as needed.Knowledge of or willingness to learn about adult day care services, community resources, and relevant regulations.Empathy, professionalism, and cultural sensitivity when interacting with diverse adult populations.Prior experience in social services, healthcare, or a related setting is preferred.High school diploma or equivalent required; additional education in social work, human services, or related fields is a plus.
