Licensed Practical Nurse (LPN) - Personal Care

Shift: Evening, 2:45pm to 11:15pm

Status: Part Time, every other weekend and holiday

Our Enhanced Living (Personal Care) team is looking for a full time nurse to join their team!

What You’ll Do

Lead and support the Neighborhood Care TeamAdminister medications and treatments accuratelyAssess and monitor resident health; communicate changes promptlyDelegate daily assignments and ensure quality, person‑centered careMentor CNAs, PCAs, and team membersCollaborate on individualized care plansEnsure regulatory and HIPAA complianceWork a flexible schedule, including weekends/holidays

Qualifications

What You Need

Graduate of an approved LPN program (RN candidates welcome)Active, unencumbered PA LPN licensePrior experience in Gerontology or Long Term Care preferred.
