Nurse Practitioner

A well established New York medical group is seeking a Nurse Practitioner who enjoys working with the geriatric population and values building long-term relationships with residents in an assisted living setting.

Positions available in:
Bronx, New York- Part Time
Port Jervis/Middletown, New York- Part Time
Dutchess County, New York- Full Time

Part-Time
2 - 3 full days per week- weekdays M-F, no weekends.
Full- time
4-5 days per week M-F , no weekends.

Nurse Practitioner Role:
Conduct full health assessments, exams, and medication reviews.
Create personalized care plans with the interdisciplinary team.
Manage chronic and acute conditions with confidence and compassion.
Order and interpret labs and diagnostic tests.
Prescribe medications responsibly within your NP scope.
Educate residents and families on staying healthy and independent.
Keep clear, up-to-date records in eClinicalWorks.
Participate in quality initiatives that truly make care better.

Required Qualifications:
Active NY State NP license.
Active DEA registration
Currently enrolled and credentialed with both Medicare and Medicaid (required).
Ability to practice independently (NPs must have a minimum of 3,600 clinical practice hours in accordance with NYS requirements).
Primary care experience required.

Competitive Compensation based on scheduling and experience.
Bonus after 90 days.
