About the role

Phoenix is seeking an experienced Program Manager to oversee residential and community-based support services for individuals with complex needs, including mental health challenges, concurrent disorders, and cognitive disabilities.

Tasks

Oversee program operations, referrals, admissions, discharges, and individualized wellness plans.
Supervise, mentor, train, and evaluate staff; coordinate scheduling and coverage.
Promote person-centred, trauma-informed, and culturally sensitive support.
Coordinate services with residents, families, community agencies, and multidisciplinary teams.
