Job description
About the role
Top Search Recruiting is seeking an RN Clinical Care Guide for our client’s primary care team in Phoenix/Scottsdale.
You’ll combine case management, patient education, referral coordination, and clinical nursing to help patients navigate their care.
Tasks
- Support patients managing chronic conditions.
- Coordinate specialist referrals, testing, and follow-up.
- Provide hands-on clinical care alongside the care team.
- Help train medical assistants and strengthen clinic processes.
Requirements
- At least two years of outpatient nursing experience and one year of case management experience.
- Lead or charge nurse experience is a plus.
