Targeted Case Manager

Somerset Mental Health is looking to hire a Targeted Case Manager.

REQUIREMENTS:
* Flexible scheduling with consideration for both employee flexibility and client needs.
* Agency-provided training in Targeted Case Management Core, Serious Emotional Disturbance (SED), and Serious Mental Illness (SMI).
* Up to six hours of annual continuing education credits available through the agency.
* Participation in two monthly meetings and supervision sessions at the main office in Somerset.
* Ability and willingness to provide services in a variety of settings, including clients' homes, the office, and schools.

QUALIFICATIONS:
* Applicants must have at least a bachelor’s degree in one of the following areas and 1 year experience post-graduation in the helping field:

(Psychology, Sociology, Social Work, Family Studies, Human Services, Counseling, Nursing, Behavioral Analysis, Public Health, Special Education, Gerontology, Recreational Therapy, Education, Occupational Therapy, Physical Therapy, Speech-Language Pathology, Rehabilitation Counseling, Faith-Based Education)
