Systemart LLC is seeking a dedicated Clinical Pharmacist to join our healthcare team in Dover, DE . This full-time position offers a unique opportunity to improve patient outcomes through expert medication management in a community-focused environment. Key Responsibilities Conduct comprehensive medication reviews and provide patient counseling on proper medication use. Collaborate with healthcare professionals to develop individualized medication therapy plans. Monitor patient outcomes and adjust medication regimens as necessary. Educate patients and healthcare staff on medication therapies, side effects, and interactions. Maintain accurate and complete patient medication records and documentation. Participate in interdisciplinary team meetings to contribute medication expertise to treatment plans. Stay current with pharmaceutical developments and best practices. Required and Preferred Qualifications Bachelor's Degree in Pharmacy required; PharmD preferred. Strong clinical skills focused on patient-centered care. 2-5 years experience in clinical pharmacy settings preferred. Current state licensure as a pharmacist. Familiarity with electronic health record systems and pharmacy management software. Board certification in a pharmacy specialty is a plus. As part of our highly qualified healthcare team, you will report directly to the Pharmacy Manager and collaborate closely with physicians, nurses, and other healthcare providers.
